New Patients are Always Welcome at Yubadocs

No matter where you live, we would love to have you as a patient. Keep in mind that a patient’s first visit generally takes longer, and it is slightly more expensive than successive visits because of all initial information that must be obtained and recorded. If you have not been seen here in more than 3 years, your visit will be considered a new visit as all your information will be updated.

If you would like to decrease the waiting time of your first visit, and you have a fax machine, please print, fill out, and fax us the patient registration form to (530) 274-7679.

If you do not fax us the form in advance, you can simply bring the completed form with you when you come in or fill it out at our office.
