After you sign in, our front office staff will request your insurance card, a photo ID (your drivers license or passport) and information about how you will make payment. You may pay for your visit with cash, check, credit or debit card. To speed up the check-in process please download and print the registration form, complete the information and bring the completed form in with you, or fax it in advance to (530) 274-7679. Don’t forget a list of medications you are taking and a list of questions you may have for the provider.

The receptionist will ask you what you are here for. This information allows us to put you in the exam room that has the equipment we need for your visit. When your name is called, you are brought back to the treatment area by a medical assistant, nurse or X-ray technician who will triage you, measure your vital signs and inquire about your medical complaint. Then a provider will come in to talk to you, examine you, and possibly order lab tests, x-rays, or treatment. If medications are prescribed, please notify the provider or medical assistant if you would like to purchase your prescription(s) on site. If you choose this option, our staff will prepare them promptly and you will pay for your visit and medicines at check out.

We will ask your permission to take your picture if you are a new patient or you have changed your appearance since your last visit. These photos do not leave our office and cannot be printed. We ask you to do this because our providers see many patients and sometimes do not complete the charting until the end of the day. Your photo gives them a visual cue to remember you and your visit and complete the note. It also helps our staff remember your name.

When you check out, you will also receive your aftercare instructions along with your prescription if you chose to purchase your prescription from us. If you prefer to have it sent to a pharmacy, the provider will usually fax your prescription directly to the pharmacy of your choice while you are still in the exam room.
